The largest expansion in Medicare since President Lyndon Johnson signed the popular health care program into law 40 years ago takes place today. That's when Medicare beneficiaries can begin signing up for the prescription drug benefit plan that becomes available Jan. 1.

The Medicare Modernization Act, passed in President Bush's first term, also includes enhanced benefits for patients with chronic illnesses and help in paying for preventive screenings. But it's the prescription drug plan that's captured everybody's attention: taxpayers, because it will cost them more than $1 trillion in the coming decade, and Part A and/or Part B Medicare clients who, if they aren't already in a prescription program, should want to sign up for Part D, and possibly save a bundle each year in prescription costs.
